What Is Silver Diamine Fluoride?

SDF is a clear liquid made of silver, water, fluoride, and ammonia. When painted onto a cavity, it does two things:

  • Kills the bacteria causing the decay (the silver component is antimicrobial)
  • Strengthens the remaining tooth structure (the fluoride remineralizes weakened enamel)

The result is that the cavity stops progressing. The treated area hardens and becomes resistant to further decay. SDF has been used in dentistry internationally for decades and was cleared by the FDA for use in the United States. The American Academy of Pediatric Dentistry and the American Dental Association both recognize SDF as an effective cavity management tool.

When Is SDF Recommended?

SDF is not a replacement for fillings or crowns in every situation. Our team recommends it in specific cases where it provides the greatest benefit:

What Happens During SDF Application

What Parents Should Know

The treated area will turn dark. This is the most important thing to understand about SDF. The silver in the solution reacts with the decayed tooth structure and turns it black. Healthy tooth structure is not affected and stays its normal color. The dark staining is permanent on that tooth but goes away when the baby tooth falls out.

For front teeth where appearance matters, we discuss this with parents beforehand. In many cases, a tooth-colored material can be placed over the darkened area after SDF treatment if cosmetics are a concern.

SDF does not repair the tooth. It arrests the cavity and prevents it from getting worse, but it does not fill in the missing tooth structure. Think of it as pressing pause on the decay process.

Reapplication may be needed. Depending on the case, we may reapply SDF at a follow-up visit (typically 2-4 weeks later, then every 6 months) to maintain its protective effect.

Does SDF hurt?
