What Is Silver Diamine Fluoride?

SDF is a clear liquid made of silver, water, fluoride, and ammonia. When painted onto a cavity, it does two things:

  • Kills the bacteria causing the decay (the silver component is antimicrobial)
  • Strengthens the remaining tooth structure (the fluoride remineralizes weakened enamel)

The result is that the cavity stops progressing. The treated area hardens and becomes resistant to further decay. SDF has been used in dentistry internationally for decades and was cleared by the FDA for use in the United States.

The American Academy of Pediatric Dentistry and the American Dental Association both recognize SDF as an effective cavity management tool.

When Is SDF Recommended?

SDF is not a replacement for fillings or crowns in every situation. Our team recommends it in specific cases:

  • Very young children (under age 3) who cannot sit through a traditional filling
  • Children with dental anxiety who need a pain-free experience
  • Children with special needs who may have difficulty tolerating longer procedures
  • Multiple small cavities that can be arrested before they require fillings
  • Temporary hold to manage a cavity until the child is ready for definitive treatment
  • Teeth that will fall out soon where a full restoration may not be necessary

For larger cavities or teeth that need structural repair, a filling or crown may still be the better option. We assess each tooth individually.

What Happens During SDF Application

The process takes about one minute per tooth. No drilling, no needles, no anesthesia.

1

Dry the Tooth

The tooth is dried with a gentle air stream.

2

Apply SDF

A small brush is used to paint the SDF liquid directly onto the cavity.

3

Wait 60 Seconds

The liquid is left to absorb for about 60 seconds.

4

Done!

That's it. Your child can eat and drink right away. Most children don't even realize a treatment took place.

What Parents Should Know

⚠️

The Treated Area Will Turn Dark

The silver reacts with decayed tooth structure and turns it black. Healthy tooth stays its normal color. The dark staining goes away when the baby tooth falls out.

🔄

SDF Does Not Repair the Tooth

It arrests the cavity and prevents it from getting worse, but does not fill in missing tooth structure. Think of it as pressing pause on the decay process.

🔁

Reapplication May Be Needed

We may reapply SDF at a follow-up visit (typically 2-4 weeks later, then every 6 months) to maintain its protective effect.

Common Questions About SDF

Does SDF hurt?

No. There is zero pain. No shots, no drilling. The liquid is simply brushed onto the tooth.

Is SDF safe for my child?

Yes. SDF has been used safely in pediatric dentistry worldwide for over 40 years. The only contraindication is a known silver allergy.

Will the staining affect the permanent tooth?

No. The staining only affects the treated baby tooth. The permanent tooth growing underneath is not impacted.

Can SDF be used on permanent teeth?

In some cases, yes. For permanent teeth, we typically prefer definitive restorations, but SDF can serve as an interim measure if needed.

How much does SDF treatment cost?

SDF is one of the most affordable dental treatments available. Many insurance plans cover it. Contact our office for specific pricing and insurance verification.
